Evidence, Experience and Engagement

Thursday June 11 2015

Waterside Inn Mississauga.

Clinical expertise and knowledge, patient values and beliefs, and best available research evidence are key components of evidence-informed decision making in all health sectors, including home care. Each of these key perspectives can enable organizations to deliver better care, better outcomes and better value. Building on areas of excellence, this one-day forum hosted by Home Care Ontario and the Canadian Home Care Association will provide an opportunity for information sharing about effective approaches to embedding evidence-informed decision making into practice at the policy, management and patient level.

Join us as we provide participants with a road map to meet this challenge!

Participants in the forum will:

Learn how high performing health systems use evidence to drive change
Gain an understanding of how patients experience and feel about their health care system

Explore innovative models and practical tools to support evidence-informed decisions

Sponsored by The Canadian Home Care Association and Home Care Ontario
