​Care Transitions are when a patient passes from the hospital to their home and it's crucial to their recuparation process, especially for vulnerable seniors with chronic ailments. 1 out of every 5 seniors is readmitted to the hospital within 30 days of initially being discharged.

A patient needs someone to make sure they take their medicines, get to the doctors and eat correctly, and assistance with those things will make a difference in the patient being successful at home or returning to the hospital.
