Job Details

Home Instead Senior Care is looking for caring and compassionate CAREGivers to become a part of our team and join our mission of enhancing the lives of aging adults throughout our community.  Home Instead Senior Care provides a variety of non-medical services that allow our clients to remain in their home and meet the challenges of aging with dignity, care and compassion.
 
Duties include, but are not limited to:

  • Companionship and conversation
  • Light housekeeping tasks and meal preparation
  • Medication and appointment reminders
  • Assistance with grooming and bathing
  • Assistance with toileting and incontinence issues
  • Alzheimer’s Care
 *No previous experience necessary – skills training provided by Home Instead Senior Care*
 
Requirements to be a CAREGiver:
  • Ability to treat and care for community members and their property with dignity and respect
  • Ability to communicate with clients in a friendly and congenial manner
  • Complete a criminal background check, a vulnerable sector check, 2 professional and 2 personal references
  • Possess a valid driver’s license is an asset, but not essential. Driver’s abstract is required.
  • Possess a vehicle with valid auto insurance is an asset but not essential.
  • Possess a computer/cell phone

Client Care Coordinator Field Nurse

Job Details

Objective:

The Field Nurse and Specialized Care Coordinator is expected to perform a variety of duties that relate to client care including taking service inquires, performing care consultations, client/CAREGiverSM introductions and quality assurance. They use the consultative sales approach to determine each individual client’s needs to provide solutions and create a tailored service plan. They continuously evaluate the plan through a series of ongoing communications and visits to ensure high quality care, client satisfaction and retention as well as opportunities to increase service hours. The Nurse will also perform “designated” nursing tasks as well as overseeing and supervising and training CAREGivers who will perform Medically Delegated Tasks.

Primary Responsibilities:                                                                             

  • Reflect the core values of 1797565 Ontario Ltd. (d.b.a. an independently owned and operated Home Instead Senior Care franchise).
  • Conduct Service Inquiries and Care Consultations with potential clients and family members to discuss their needs and provide solutions in the form of a care plan.
  • Meet with staff at Long-term care homes to review care plans of clients and work with the scheduling team and CAREGivers to assign and train staff as appropriate
  • Work with Long-term care home staff, clients and their families on the various issues that may arise to ensure they are getting resolution to issues as well as receiving the services needed.
  • Conduct client/CAREGiver introductions with every new client and with every new CAREGiver.
  • Work with other team members to coordinate various aspects of a client’s care.
  • Create and maintain client and responsible party records documenting all quality assurance meetings.
  • Conduct and coordinate training programs related to specialized care, nursing and regulated acts
  • Develop and maintain Policies and Procedures regarding Medically Delegated Tasks
  • Arrange and/or provide client care by utilizing the skills of observation, assessment, nursing diagnosis, counseling and health teaching; reporting as necessary
  • Provide care according to the established care plan ensuring that, on an ongoing basis, the emotional, spiritual, physical comfort and safety of clients are met to the greatest possible extent; care must be within the scope of practice of a Registered Nurse.
  • Perform routine nursing duties in accordance with company policies and procedures and Standards of Nursing Practice as defined by the provincial regulating body; perform specialized nursing procedures for which recognized training and current competency can be demonstrated.
  • Receive and transcribe physician’s orders.
  • Provide direction, support and delegation to Licensed/Registered Practical Nurses and CAREGivers as appropriate.
  • Demonstrate open and effective communication with Long-term care home staff, clients, family members, the franchise owner, colleagues and CAREGivers, and referral providers/care providers.
  • Maintain regular attendance at the office to execute job responsibilities.
  • Adhere to all company policies, procedures and business ethics codes and ensures that they are communicated and implemented within the team.

Secondary Responsibilities

  • Participate as needed in all CAREGiver meetings
  • Conduct Family Education sessions as needed
  • Perform all other functions deemed necessary
Education/Experience Requirements:
  • Registered Practical Nurse, licensed and registered with the College of Nurses of Ontario, in good standing
  • Demonstrated competence within the full scope of practice of a Registered Practical Nurse
  • Current membership with the Registered Practical Nurses Association of Ontario
  • Knowledge of College of Nurses of Ontario (CNO) Standards of Practice and commitment to maintaining reflective practice standards as described by the CNO
  • Experience in the delivery of primary care with ability to provide education
  • Five years+ experience in home care, health care or senior-related industry required, an equivalent combination of education and work experience may be considered.
  • Must possess a valid driver’s license

Supervisory Responsibilities:

  • CAREGivers providing regulated acts or nursing

Knowledge, Skills and Abilities:

  • Must have an understanding of and uphold the policies and procedures established by 1797565 Ontario Ltd. (d.b.a. an independently owned and operated Home Instead Senior Care franchise)
  • Must demonstrate excellent oral and written communication skills and the ability to listen effectively
  • Must have the ability to work independently, maintain confidentiality of information and meet deadlines
  • Must demonstrate effective interpersonal skills as well as sound judgment and good decision-making skills
  • Must demonstrate discretion, integrity and fair-mindedness consistent with office standards, practices, policies and procedures
  • Must demonstrate knowledge of the senior care industry
  • Must have the ability to organize and prioritize daily, monthly, quarterly and yearly work
  • Must have the ability to establish good working relationships with the franchise owner, office colleagues, CAREGivers and the community
  • Must have the ability to present a professional appearance and demeanor
  • Must have the ability to operate office equipment
  • Must be patient and congenial on the telephone 
  • Must have computer skills and be proficient in Excel and Word
  • Must have the availability to work evenings or weekends as required
  • Must have the ability to perform duties in a professional office setting and work as part of a team
