Client Care Coordinator

We are hiring! We are in need of a full time Client Care Coordinator for our office, hours are Monday - Friday from 9-5! This individual must be a self-starter, have excellent relationship management skills and be adaptable to the ever-changing senior care industry. This position involves managing quality assurance with extensive one-on-one interaction with clients and families. This person is responsible for effectively managing multiple demands that come with the changing needs of clients. This individual must be able to work in a fast-paced organization and possess excellent communication skills, be service-oriented and have the ability to work well in a team environment.

Primary Responsibilities:
• Reflect the Core Values, Mission, and Mission Statement of our company.
• Meet with potential clients and family members to discuss their needs and provide solutions in the form of a care plan.
• Conduct Service Inquiries and Care Consultations as needed following the consultative sales process
• Maintain ongoing communication and a consistent follow-up schedule with potential clients and families who have yet to commit to service.
• Work with other team members to coordinate various aspects of a client’s care.
• Conduct client/CAREGiver introductions with every new client and with every new CAREGiver.
• Create and maintain client and responsible party records documenting all quality assurance touchpoints.
• Enter and maintain accurate client and CAREGiver records in our software (entering notes, updates, tasks, and pertinent information)
• Review client list weekly and report on clients lost/deactivated.
• Prepare and update Care Consultation folders if needed or if you see an update that needs to happen.
• Update Client Journals and Care Plans
• Prepare Client Journal for new clients if needed.
• Schedule and attend monthly one-on-one meeting with supervisor.
• Update Client Journal with appropriate and updated/revised documentation
• Perform regular networking visits and manage contacts with all DOC, ED, SW, and staff at facilities where we have clients (Retirement Homes, LTC, Rehab, CCAC).
• Use the consultative sales process to maximize existing referral relationships as well as seek out and establish new referral relationships.
• Develop and implement quality improvements and innovations to increase quality of care and client experience.
• Plan, execute, and document QA Schedule ensuring a QA visit/call is performed for all clients.
• Follow-up with clients and family contacts after first shift in cooperation with Quality Assurance Specialist and CAREGiver Supervisor.
• Create, maintain, and update all client files.
• Recognize and pursue opportunities to modify care plans to best support the ongoing needs of clients through the quality assurance program.
• Increase client loyalty to Home Instead Senior Care by utilizing the consultative sales process to better meet our current client needs.
• Recognize and capture opportunities to increase service hours in scenarios to enhance and/or increase quality care.
• Demonstrate open and effective communication with clients, family members, the franchise owner, colleagues and CAREGivers, and referral providers/care providers.
• Work with clients and their families on the various issues that may arise to ensure they are getting resolution to issues as well as receiving the services needed.
• Demonstrate open and effective communication with the franchise owner, colleagues, CAREGivers, clients and family members.
• Maintain regular attendance at the office to execute job responsibilities.
• Adhere to all company policies, procedures and business ethics codes and ensures that they are communicated and implemented within the team.
• Communicate client and CAREGiver concerns or problems with owner or other staff members as appropriate
• Demonstrate open, positive, and effective communication with owners, colleagues, Clients and CAREGivers
