Meet Our Home Care Team

Jake Mitten, Owner and Managing Director

Jake Mitten, Owner and Managing Director

​Jake has always enjoyed taking on new challenges in both his professional and personal life, which ultimately drove him to opening a Home Instead business. Working in a fast-paced sales environment, Jake developed the skills to build effective relationships and solve problems in collaborative and creative ways. A graduate of the Bachelor of Business Administration (BBA) program at Wilfrid Laurier University, Jake is passionate about making a positive difference in peoples’ lives, and is truly thrilled he now has the opportunity to help others in the community through his work at Home Instead. Loyalty and patience are two of his key traits – evident in the fact he remains a lifelong fan of the Toronto Maple Leafs.

Tracy Mitten, RN, Owner and Director of Client Care

A passionate nurse for over 30 years, Tracy knew from a young age she wanted a career where she could make a positive impact in peoples’ lives. Throughout her career, Tracy has worked in a variety of nursing fields, including nephrology (providing hemo and peritoneal dialysis) and chronic disease management.

Most recently, Tracy has spent the last 10 years working in a geriatric outreach program and with Behavioural Supports Ontario managing behaviours associated with dementia and other neurological conditions for individuals living in long term care. Through this experience, Tracy witnessed the challenges families face in caring for their aging loved ones first-hand, and clearly saw the need for improved in-home senior care. Tracy is excited to make a difference in the lives of seniors and their families and believes Home Instead’s mission aligns directly with her vision for enhancing the lives of older adults. She is a graduate from Ryerson University’s BSc. N program, which she completed part-time while raising a family of three children

Cedricka Johnson, Recruitment and Engagement Coordinator

A people person by nature, getting into Human Resources was the obvious choice for Cedricka who wanted to pursue her passion of helping individuals reach their full potential. She recently became a Bachelor of Commerce graduate from Ryerson University (specializing in HR) where she completed the majority of her studies while working full time in the industry. Her Recruitment and Engagement Coordinator role is focused on building our team of excellent CAREGivers and making Home Instead Markham a top organization in the community. Cedricka takes pride in the fact that the CAREGivers she hires for our team are directly responsible for helping local seniors who truly need our support. Outside of work you can find her hiking new trails, binge watching the latest series on Netflix, or sipping lattes at her local Starbucks.
Cheryl Hartling, Service Coordinator

Cheryl Hartling, Service Coordinator

Throughout her career Cheryl has held several roles focused on providing top quality client service. She thrived in these environments where she could utilize her people and problem solving skills to help others. Cheryl has always been big on giving back to the community and wanted to be able to do so in her career. This is what brought her to the senior care industry and ultimately to Home Instead a few years later. As our Service Coordinator, she is responsible for scheduling clients and CAREGivers to create extraordinary relationships as well as being the initial point of contact for anyone calling into the office. What she loves most about her job is being able to build strong relationships with both the families we serve and our staff in the field. When Cheryl is not hard at work she enjoys spending time with family, reading, and volunteering at her church. In the summer you can even catch her relaxing at her up north getaway!
