Home Care Case Manager position in Pasadena, CA with base salary ranging from $54K-$74K, healthcare benefits, paid vacation, 401K, and company vehicle. Home Instead Pasadena is seeking a driven case manager to help us change the face of aging in our community.
The Home Care Case Manager (i.e., Client Care Coordinator or CCC) is responsible for development and implementation of our client’s “person centered” plan of care. The CCC will ensure that all Care Professionals working with their assigned cases have the skills and in-field training necessary to ensure positive quality outcomes. The CCC will address emerging issues in a timely manner. This role has some community outreach responsibilities and staff training opportunities. The CCC position requires a valid California drivers license and automobile insurance. We are looking for individuals with at least 3 years of experience in home care case management. A background in nursing or gerontology and fluent Mandarin or Cantonese is a plus. This full-time position is scheduled Sunday-Thursday or Tuesday-Saturday with start times based on business needs.
