The Opportunity

Home Instead Markham is looking for a dedicated and ambitious RPN or RN to join our dynamic team as a Client Care Coordinator. We are a rapidly growing office that opened in 2020 with a clear goal: to empower every aging adult in our community to remain at home with independence, dignity, and the support they deserve. As demand for our services continues to rise, we're seeking someone who shares our passion for exceptional in-home senior care and is excited to help ensure every client receives high-quality, personalized care.

This is a high-impact role for a nurse who thrives on building strong client relationships, supporting caregivers, and ensuring care outcomes are met. You’ll be a trusted advisor for clients and families, oversee the delivery of care (including our Nurse Directed Services program), train and mentor Care Professionals, and play a key role in maintaining client satisfaction and retention. Key Responsibilities

  • In-Home Consultations & Client Onboarding: Conduct in-home assessments, build trust with clients and families, guide them through the decision to start care, and oversee a smooth, effective onboarding into services, including care plan development.
  • Client Advisor: Serve as the main point of contact for clients and families, providing guidance, support, and solutions to meet their care needs.
  • Quality Oversight: Check in regularly with clients (both in person and by phone/email) to ensure care is delivered as planned, identify issues early, and resolve concerns quickly.
  • Care Coordination: Work closely with the scheduling and caregiving teams to ensure smooth care delivery and strong caregiver-client matches.
  • Coaching & Mentoring: Train, guide, and provide feedback to Care Professionals to maintain high standards and consistent service.
  • Nurse Directed Services Oversight: Oversee and delegate enhanced care services, supporting Care Professionals in delivering specialized clinical care safely and effectively.
  • Client Problem Resolution: Anticipate client needs, address challenges, and provide practical solutions to support positive outcomes.
  • Team Collaboration: Work with leadership and broader team to improve client experience, processes, and support office goals.
  • Documentation: Keep accurate records of client interactions, care plans, and quality checks in internal systems.
  • Client Retention & Growth: Strengthen client trust and satisfaction, and help grow the business through exceptional service and follow-up.
  • Care Plan Adaption: Adjust to changes in client needs, schedules, or care requirements to ensure smooth service delivery.
  • Inquiry and Intake Support: Respond to incoming calls and inquiries from families, provide clear information about our services, answer questions, and guide prospective clients through the early intake process.
  • Special Projects: Pitch in on cross-functional initiatives and internal projects as needed — we’re a growing small business, and every role plays a part in our success.

Required Qualifications

  • Current Registered Practical Nurse (RPN) or Registered Nurse (RN) license in good standing to practice in Ontario.
  • Minimum 5 years of nursing experience.
  • Strong client relationship skills and ability to build trust with clients, families, and Care Professionals.
  • High proficiency with technology, including Microsoft Office Suite and ChatGPT.
  • Excellent verbal and written communication skills.
  • Valid G-level driver’s license, full-time access to a reliable vehicle, and up-to-date car insurance.

Preferred Qualifications

  • Minimum 2 years of experience working in home care and/or senior care.
  • Familiarity with the home care and retirement living landscape in Ontario.
  • Proficiency in Mandarin and/or Cantonese would be a nice to have, but is absolutely not a requirement.

This role will mostly be Monday to Friday with some evening and weekend work required. You will be working throughout the community in Markham, at our office at Highway 7 and Woodbine, and at home.
