Client Care Coordinator

Home Instead is an industry leader in providing non-medical home support to seniors around the globe. We have an exciting opportunity for a Client Care Coordinator to join us in our mission to enhance the lives of aging adults and their families.

The position of Client Care Coordinator performs a variety of duties including consulting with potential clients and family members, developing individualized care plans, introducing CAREGivers to clients, conducting regular quality assurance visits, documenting and updating care plans to ensure all aspects of client needs are met.

The Client Care Coordinator promotes our services and develops strong relationships with health care providers and others in the community who support and advocate for seniors in our community. Primary Responsibilities:

  • Consult, create and provide assessments and care plans to seniors and families in our care
  • Ability to communicate clearly and listen effectively inside and outside of the organization
  • Ongoing collaboration and follow-up with families, medical and other professionals
  • Communicate effectively, both verbally and in writing
  • Organize, prioritize and coordinate workload
  • Foster good public relations
  • Work independently and as a member of, and/or provide leadership to team
  • Documentation and report writing

What You Will Bring to the Role:

  • A collaborative nature
  • The ability to also work independently, be self-motivated and be proactive
  • Excellent oral and written communication skills and being comfortable with making presentations
  • Strong interpersonal skills, sound judgment and good decision-making skills
  • The ability to set goals and achieve them in a timely manner
  • Excellent prioritization, time management skills and comfortable with technology
  • A professional appearance and demeanor
  • A medical background or knowledge in the field is a strong asset
  • Must possess a valid driver’s license

Ideal candidates will have knowledge of and/or experience working with:

  • People with Alzheimer’s and other dementia
  • Understanding of the needs of older adults
  • Nursing or other medical background is a plus

Education and Experience

  • Background in nursing (LPN degree), social work or relevant work experience in home care, health care or senior related industry required or equivalent combination of education and work experience
  • Proven success in a business development function (sales and customer service-related industry)
  • Must possess a valid driver’s license
