We are looking for a Full-Time Client Care Coordinator to join our growing team! 

The Client Care Coordinator is expected to perform a variety of duties that relate to client care and service delivery for our clients. They are expected to be responsible for providing the highest level of quality service to all clients and family members. We will be able to achieve this by developing relationships with those individuals that make all of this possible – the CAREGivers! Also, working well with team members and by being commitment to quality service. In addition, the Client Care Coordinator will work hand-in-hand with the Mississauga/Etobicoke team to ensure a positive, cohesive and supportive environment for all Key Players to ensure that all needs of our team are met.

  • Ensure the needs of the client, family, employee, and company are consistently being met
  • Meet with potential clients and family members to discuss their needs and provide solutions in the form of a plan of care
  • Work with other team members to coordinate various aspects of a client’s care
  • Complete Care Consultations for clients, prepare the care plan & enter and maintain client records in our operating system
  • Conduct client/CAREGiver introductions with every new client
  • Develop a Quality Assurance frequency plan which will be approved by the Director of Client Care
  • Execute the Quality Assurance frequency plan to ensure each client has QA visits and calls as per the plan
  • Recognize and pursue opportunities to modify service plans to best support the ongoing needs of clients through Quality Assurance visits
  • Demonstrate open and effective communication with Director of Client Care, Managing Director, colleagues and CAREGivers
  • Meet weekly for team meeting and client care meeting where previous weeks Care Consultations and Quality Assurance visits will be discussed along with critical numbers
  • Ability to work independently, maintain confidentiality of information and meet deadlines
  • Demonstrate consistent logic, rationality, objectivity and common sense in decision making while anticipating the consequences of decisions.
  • Discerns when there is a need for quick decisions or more thorough assessments
  • Generate new & creative approaches to problems or original modifications (innovations) to established approach
  • Share your heart by participate in the CAREGiver retention programs and coordinate Guiding Principle winners for the monthly newsletter 
  • Provide assistance to various members of the team as needed
  • Embrace the 5 Dysfunctions team model and continually apply the lessons to the team ensuring a cohesive & unified team
  • Answer each incoming call in a friendly, professional and knowledgeable manner
  • Field new client inquires over the phone in a knowledgeable manner, enter information into CLEAR CARE
  • Ability maintain confidentiality of information
  • Maintain accurate client records in CLEAR CARE
  • Monitor, mediate, and log both client and CAREGiver activity and follow up on client service in CLEAR CARE
  • Reflect the core values & Guiding Principles of 6748627 Canada Inc./702244 Ontario Ltd., (d.b.a. an independently owned and operated Home Instead Senior Care franchise)
  • Days of work will either be Sunday to Thursday or Tuesday to Saturday
  • Hours of work will be 8:30am-5:00pm
  • Once training is completed, will be part of the on-call schedule
